Information management

1. Information report

For confirmed cases, medical institutions should fill in the disease development information through the H7N9 bird flu information management system and the online H7N9 Bird Flue Questionnaire Form—Clinical Section within 24 hours after the patient is discharged or dead (more details in the H7N9 Bird Flu Epidemiological Investigation Plan). In cases of death, medical institutes should carefully fill in death certificate and make a direct network report through the cause of death registration and report information system. The local disease control and prevention center at the county level should fill in the online H7N9 Bird Flue Questionnaire Form—Epidemic Section after preliminary investigation (more details in the H7N9 Bird Flu Epidemiological Investigation Plan) and update the questionnaire form based on daily medical observation of close contacts.

If the direct network report case switches hospitals, the original medical institute should record the case transfer in the H7N9 bird flu information management system. The medical institute taking in the case should verify the case information through the system and put in the case admittance and treatment development.

Clustering cases should be directly reported online through the public health emergency report management system two hours after they’re confirmed. Progress and conclusive reports should be made based on case development.

Disease control and prevention center carrying out lab tests should immediately put sample information and test results in the national flu monitoring information system.

Provinces failing to deliver information report and sample (strains) as required will be punished by the National Health and Family Planning Commission.

Information release and report

The National Health and Family Planning Commission regularly releases H7N9 infection and mortality numbers nationwide every month, and the provincial health and family planning administrative department releases local information.

The local disease control and prevention center should report the external environment sample with positive results to the local health and family planning administrative department and higher disease control and prevention center. The health and family planning administrative department should report it to the agricultural department at the same level.

The national joint defense and control expert team should review and release information if the virus mutates or human-to-human transmission occurs.
